Athletic Eligibility Packets are now available for the 2004-2005 school
year. You may download an Adobe acrobat version, fill in the
blanks on your computer, and print it out. There is 7 pages total, some pages
will require you to check and write things, many of the blanks that are for names
and other information may be typed on the form and printed out before you
take it to your doctor. You will use the tab key to go from field to field.

After downloading Acrobat now you can download your Athletic Packet.
This file is approx 1.6 mb, highspeed connection is recommended. A modem
connection could take a very long time (+1 hour). You may pick up a printed
copy of the packet in the main office.
